If you do not meet any of the above then you will be in a large amount of pain in doing so. Especially the remote parties problem will cause a lot amount of pain as they need to do it in sync with you and they need to find out where all the changes have to be made.
There's an account of renumbering experiences here from work we did last year: http://www.6net.org/publications/deliverables/D3.6.1.pdf and http://www.6net.org/publications/deliverables/D3.6.2.pdf There's a lot of recommendations (20 or more) on things that would be useful to aid renumbering further. But the basic network oriented approach using prefix advertisements and deprecation works across the operating systems tested. As Jeroen says though, there's a wee bit more to it than that. Perhaps most importantly network management tools need updating to understand that IPv6 hosts may be multi-addressed, and to be able to validate/verify the phases of the renumbering process. -- Tim/::1
